Expanding Donor Possibilities in Hair Restoration

The BHT technique — Body Hair Transplant — is a specialized variation of the FUE method, in which donor follicles are extracted from body areas rather than the scalp. While the extraction principles are similar, BHT is reserved for specific, carefully selected cases.

This technique is primarily used when a patient has insufficient donor supply in the scalp to achieve the desired coverage or density. In such cases, body hair may be used as a complementary donor source, rather than a complete substitute.

Biological Considerations & Limitations

Body hair differs significantly from scalp hair in terms of growth rate, follicle type, texture, and survival rate after transplantation. Most body hair follicular units are single-hair grafts, which means they offer lower density potential. Additionally, body hair has a shorter anagen (growth) phase, which limits its capacity to grow long or remain in active growth for extended periods.

However, due to a phenomenon known as “recipient site dominance,” transplanted body hair may perform better in the scalp than it would in its original location. This is thought to be influenced by the vascular environment and other local factors in the scalp.

Even with these benefits, the expected length of growth for body hair on the scalp typically does not exceed 5 cm.

Beard Hair – The Most Reliable Body Donor Source

Among all body hair donor sites, the beard offers the best results in terms of survival rate, texture, and yield. Beard follicles are often grouped in twos and are generally thicker, providing improved coverage compared to other body areas.

Beard hair also tends to grow longer than other types of body hair, and it has shown a survival rate comparable to or even higher than scalp hair. For these reasons, beard hair is occasionally used in non-visible areas, such as the crown or mid-scalp, but not in the frontal hairline, where its texture and density would not match natural hairline aesthetics.

Strategic Application & Patient Selection

Because body and beard hair differ in texture, growth pattern, and color, compatibility is essential. Before proceeding with a large BHT session, a small test transplant is often performed to evaluate graft performance and aesthetic integration.

BHT is not a standard approach and should be considered only in carefully evaluated cases. Its success relies heavily on the surgeon’s experience, donor hair characteristics, and realistic patient expectations.
