Techniques Used in Repair Procedures
✔️ Camouflaging or Replacing Large Grafts (“Plugs”)
In cases where old grafts (plugs) are large and poorly placed — especially in the hairline — they can be surgically extracted and reused. The follicles are then divided into individual follicular units under a microscope and redistributed to more natural locations. In selected patients, this technique can eliminate the “pluggy” look in a single session.
If the grafts are not excessively large, they may be camouflaged by placing refined follicular unit grafts around them to soften their appearance and blend with the surrounding hair.
✔️ Correcting Hair Direction
One of the most visually disturbing problems is unnatural hair direction, especially in the frontal zone. Patients often struggle to style their hair and feel self-conscious due to odd angles or vertical growth.
This can be addressed by extracting misaligned grafts and repositioning them at the correct angle and direction. Simply adding new grafts without removing the old ones can result in a confusing mix of orientations, which still looks unnatural.
✔️ Repairing Skin Damage in the Recipient Area
Unnatural textures and scarring on the scalp may result from past procedures. Common conditions include:
- Pitting: Small depressions caused by grafts placed too deeply
- Cobblestoning: Raised bumps due to excessive trauma or large incisions
- Tenting: Minor bumps at the exit point of each follicle
- Ridging: Raised scar lines due to dense packing of grafts with excess dermal tissue
Depending on the case, these issues can be treated with a combination of camouflage, surgical extraction, reconstruction, laser resurfacing, or corticosteroid injections. Results vary based on severity and skin type.
✔️ Redesigning the Hairline
Many repair patients suffer from hairlines that are too straight, overly dense, or visibly artificial. A natural hairline is irregular, asymmetrical, and gradually transitions from fine single-hair grafts in the front to higher densities further back.
Using refined follicular unit grafts placed at the proper angles and depths, we can reconstruct a soft, natural-looking hairline that blends seamlessly with the patient’s features.

✔️ Donor Area Repair After Poor FUE Extraction
Excessive or uneven harvesting during a previous FUE procedure can leave visible thinning or patchy bald spots in the donor area. This often occurs when extractions are concentrated in limited zones instead of being evenly distributed.
In some cases, we can redistribute grafts to improve coverage, although overharvested donor areas may not be fully repairable.

Corrective surgery is often more complex and costly than the original procedure. Factors such as scar tissue, fibrosis, poor vascularization, or depleted donor supply can make the process more difficult or, in some cases, unfeasible.
When the donor area has been severely overused, further transplantation may not be an option.
